Instructions:

• After completing and submitting Section 2, open Section 3: Choose your sponsorship level add-ons. When choosing, remember that the total value of your selected sponsorship add-on opportunities should add up to no more than the total value of your chosen sponsorship level. Please complete the authorization contact information before submitting this agreement.
