INSTRUCTIONS FOR NEW REGISTRATIONS

Online registration for e-Health 2008 s now closed. Please register using the PDF form.

Fax your completed form to 604-685-3521 until Friday May 2nd, 2008, 5:00PM Pacific Time. After this date, please visit the onsite e-Health registration desk, located at the main entrance of the Vancouver Convention and Exhibitor, 999, Canada Place Way.

If registering onsite, please help speed up the process by filling in your registration form before coming to the onsite registration desk.

NOTE: Cancellations at this date are now 100% non-refundable. Substitutions can be made and are subject to a $50 plus taxes administration fee.
